Complete your paperwork ahead of time to save waiting time at the clinic! If you are unable to print out the paperwork ahead of time, please arrive 10-15 minutes prior to your appointment.

Insurance
Please bring your proof of insurance with you to your first visit (excluding Worker’s Compensation). We accept most insurance—please call to verify. Choosing not to go through insurance? Inquire with our office for discounted rates for self/cash pay.

Family/Caregiver
We welcome family or caregivers to attend all appointments. While we try to limit space in the gymnasium, there is a waiting area provided.
